HeartLink: When You Need Us Most

You are so tired and not looking forward to another drive in the snow and a two-hour wait in the emergency room at the hospital – for the third time this month. You’re so scared, though, that you can’t breathe right, and don’t feel that you have any other choices.

Crossroads Hospice can help.

HeartLink is a unique program offered by Crossroads Hospice to meet the needs of patients in the final stages of heart disease. A wide range of symptoms can present themselves with heart disease, often unpredictably. Crossroads is fully prepared to manage all of the patient’s emotional and physical symptoms as they arise. We have trained nurses, nurse practioners, and physicians on staff who have high-level knowledge of the needs of heart patients, including the best options for symptom management.

Crossroads makes every attempt to care for patients in the environment of their choice, usually their own home, so they don’t have to make continual, stressful hospital visits. We also have staff available to sit with a patient at the time leading up to their death and trained grief support staff available to assist the grieving family following the death of the patient.
