The National Quality Forum is a nonprofit, nonpartisan, public service organization committed to improving the nation’s healthcare through the use of standardized healthcare performance measures.

This webinar will educate long-term care providers on the accepted measures of the National Quality Forum, the reporting of those measures by hospice, and how it can affect hospice care residents. Implications of performance measures can drastically affect delivery of care, and long-term care facilities may face scrutiny from inspectors over deficiencies.

Perry Farmer is president, and an original founder, of Crossroads Hospice. Driven by creativity and enthusiasm, Farmer started his career in healthcare in 1989. Believing in learning from the ground up, Farmer began as a nurse’s aide and worked through each department in the nursing home, eventually becoming administrator.

In 1995, Farmer and his family started Crossroads Hospice. Learning from his long-term care background, their program has changed expectations of hospice care in long-term care facilities in each of the communities they serve.
