Evenmore Care
Our commitment to patient care is probably best demonstrated by our Evenmore Care© program. Evenmore Care was developed to ensure every patient receives what they deserve…more support, more time, more attention.
How does it work? Through our lower-than-average patient-to-staff ratios, we have the ability to see patients virtually every day. With a higher level of patient contact, our staff can better identify an increased or sudden decline in a patient’s condition. Once a notable change in condition is identified, we initiate our Watch Program©.
The Watch Program is a special care level created to monitor a patient’s developments through multiple daily visits. Because Crossroads staff is there several times throughout the day, we can better recognize when the passing of a patient may be imminent. At that point, we exhaust every effort to stay by their side.
This intense level of care was developed so we can be there when patients and families need us most. And best of all, it’s extremely effective. Evenmore Care and the Watch Program have enabled Crossroads Hospice to attend the time of passing at four to five times the national average. Something our patients and their families value highly and our healthcare partners have come to depend on.

Veteran’s Recognition
Throughout history thousands of men and women have served in the United States armed forces, putting aside regard for themselves and leaving behind family and friends to defend our great nation and its ideals. We recognize the sacrifices these men and women have made and believe we owe a debt of gratitude for the security they have provided by defending our nation.
We also understand the traumatic effects war can have on veterans and the feelings it can produce like anxiety, depression, reoccurring anger, self-blame, guilt, compulsive or aggressive behavior, and sleep disorders. This is known as Post-Traumatic Stress Disorder (PTSD). Some are affected so strongly that they are unable to live normal lives; and some may not experience any symptoms until they approach the end of life.
To better care for these patients, the Crossroads Hospice staff and volunteers are educated and trained in caring for veterans at the end-of-life. As a part of training, staff members are educated in PTSD and how it affects dying for certain veterans.
Additionally, we have developed a “Veteran Recognition Program”. The purpose is to acknowledge and recognize our veteran patients. These patients receive a certificate of appreciation, a medal of honor and a veteran’s cap. In addition, our volunteers record the veteran’s experiences in journals we prepare for their families.

Hispanic Outreach
Our Hispanic Outreach Program was designed to meet the needs of the Latino families we serve. Bi-lingual employees are specially trained to work with patients to develop unique care plans by incorporating cultural and other needs of the patient and family. Good communication is an important aspect of patient care, so we are able to communicate in a manner that is most comfortable for everyone. We also offer our literature and all critical documentation in Spanish to ensure everyone is on the same page.

Grief Recovery
We offer families comprehensive bereavement and grief counseling; and we know that grief can’t be put on a timetable, so we’re there for as long as you need us.
While grief is normal and natural, most of the information passed on within our society about dealing with grief is not. Grief is the emotional response to a loss of any kind. However, most of the information we have learned about dealing with loss is intellectual and incorrect. We hear things such as: time heals all wounds; don’t feel bad; grieve alone; replace the loss; or keep busy. In reality, recovery from loss is accomplished by discovering and completing all of the undelivered communications that accrue in relationships.
The Grief Recovery® Program teaches choices that help people “move beyond” the pain caused by loss. It is a twelve-week program led by a Crossroads’ employee who has been trained and certified by the Grief Recovery® Institute. It creates a safe environment in which to look at old beliefs about dealing with loss; what other losses have affected your life; and take new actions that lead to the completion of the pain attached to a loss. It is offered at no charge and is available to anyone in need of grief support.

Memorials
Crossroads holds memorial service to honor the memory of the patients we have served. Families are invited to attend the services to celebrate the life of their loved one.
