Partnering For Better Care In Our Communities.
ER frequent flyers. We’ve all experienced them, the same community members requiring emergency services again and again, with not much first responders or even a medical team can really do for them.
Often these frequent flyers are people with chronic illnesses that have advanced to an end-stage. When these patients call repeatedly for emergency support, it is likely time for the patient and family to consider hospice care.
Crossroads welcomes the opportunity to meet with you and your coworkers to explain what hospice care can provide to improve quality of life for these community members. If they do not qualify for hospice care, there are other services we can identify to keep those frequent flyers home and free of distress.
