Hospice Admission Checklist 

Transitioning a loved one into comfort care requires gathering important information so the care team can begin support without delay.  

Creating a hospice admission checklist helps families organize the necessary administrative, financial, and medical details before the initial visit. Knowing what to have ready allows the admission process to go smoothly. 

ID and insurance

The first part of a hospice care checklist involves identification and insurance details. Families should gather the patient's:  

  • Full name 
  • Date of birth 
  • Current address 
  • Emergency contact information 

It’s also necessary to compile all insurance cards including Medicare, Medicaid, or private health insurance information. Having these documents ready allows the billing coordinators at Crossroads Hospice and Palliative Care to quickly verify benefits and ensure that 100% coverage for hospice related fees can be activated. 

Medical  

The medical portion of the hospice admission checklist focuses on the patient’s current health status and treatments. Caregivers should prepare a complete list of current medications and dosages.  

Keeping the original prescription bottles handy allows the admitting nurse to review all medications for pain and symptom management. You should also note the names and contact details of the patient primary care physician and specialists, along with information on recent hospital admissions or emergency room visits. 

Legal

Finally, a complete checklist for hospice admission must include critical legal documents and care preferences.  

  • Families should locate copies of: 
  • Advanced medical directives 
  • Living wills 
  • Documentation showing who should make decisions if the patient is no longer able to do so
