The Crossroads Difference: Evenmore Care

Evenmore care

Patients are eligible to begin hospice care when they have been diagnosed with a terminal illness with a life expectancy of six months or less if the illness follows its typical trajectory. At Crossroads Hospice & Palliative Care, we take pride in supporting patients and families through their final months, weeks, and days, but it’s our Evenmore Care program in the patient’s final hours that we are most proud of.

At Crossroads, we operate with two core beliefs:

  1. No one should die in pain.
  2. No one should die alone.

As a person begins to exhibit the signs and symptoms that end of life is approaching, our hospice care team increases their visits to meet the patient’s needs in the patient’s final seven days of life. When it appears that death is imminent, our care team begins Evenmore Care, remaining at the patient’s bedside to support them and their family through their final hours.

Support When You Need it Most

Every patient is unique and their final hours will also be unique. Some will pass quickly. Others may decline over several days or hours.

During this period, you may observe:

  • Decreased blood pressure
  • Irregular breathing
  • A surge of energy
  • Glassy or half-opened eyes
  • Purple mottling on the hands and feet
  • Increased restlessness
  • Decreased urine output
  • Weak pulse

It’s normal for a loved one to feel many emotions when a loved one is dying. You may have questions about their breathing patterns or other changes in their condition. Our Evenmore Care team is trained to provide emotional support, answer your questions, and provide peace of mind.

Some loved ones want to remain in the room, providing their own presence and support to their loved one. Your hospice care team will provide care alongside you.

Other loved ones may need a break or may be unable to visit in person. A member of our team will remain bedside, providing your loved one with any support that is needed.

After Death Occurs 

Once your loved one passes away, you may wish to continue to spend some time with their body. Some sit quietly. Some share stories or perform religious rituals. You may wish to leave the room for time alone or to make calls notifying friends and family. There is no right or wrong emotional response.

Our Evenmore Care team will provide aftercare, ensure that the death is officially pronounced by the proper authorities and can notify the funeral home of the death. We’re there to support you through the process.
