June is Alzheimer's & Brain Awareness Month.
Individuals with dementia and Alzheimer's diagnoses are one of the fastest growing groups of hospice users in America. Over 5 million Americans are currently living with Alzheimer’s disease and related dementias, with more than 15 million family caregivers providing unpaid support. Understanding hospice eligibility for Alzheimer's/dementia is crucial for these families, as it helps determine when their loved one can receive specialized care hospice offers.
For the month of June we turn our signature green purple to help bring awareness and demystify the sixth leading cause of death for Americans.

Criteria for Hospice Questions Check Eligibility
Support Does More the Sooner You Have It.
Due to the slow progression of Alzheimer’s and other dementias, it can be difficult for family members to determine when a patient becomes eligible for hospice care. That’'s why it’s important to start the conversation early – so you have a plan in place should hospice eligibility criteria be met.
If you’re unsure whether hospice care is the right choice, take a hospice quiz to help assess the situation and determine if hospice services could provide the comfort and support your loved one needs.
Crossroads Hospice & Palliative Care is also available to assess the patient in their home environment — including private residences, hospitals, assisted living facilities, etc. — to determine whether they meet the hospice care requirements for dementia. We are happy to meet with families at the time and place most convenient for them.
To arrange a hospice consultation, please contact us at 855-327-4677. Our team is available 24 hours a day to take your call.
Care Patients Receive.
Hospice services are delivered wherever the patient calls home. If you wish to remain in your family home, we’ll come there. If you’re in an assisted living facility or nursing home, we can partner with the facility to provide extra, specialized end-of-life care.
While receiving hospice care, patients receive visits from their personal team of healthcare professionals including nurses, aides, social workers, chaplains, and volunteers. Each has been trained to meet the patient’s physical, mental, emotional, spiritual, and personal care needs. Your hospice team will help with bathing, administering medication, wound care, companionship, and more.
In addition, all medication, medical equipment, and medical supplies related to your diagnosis will be provided at no cost to you or your family. Contact us now for additional hospice information for patients.
