Families can need the extra care of hospice at any time. Medical emergencies happen in the middle of the night. Patients are discharged on holidays. You can’t expect 9-to-5 hospice care to meet 24/7 healthcare needs.

When you call Crossroads, our Quick Response Team springs into action to ensure your loved one is quickly assessed and admitted to our care if they meet hospice criteria.

Available 24 hours a day, we have a dedicated Quick Response Team ready to admit patients and are staffed to provide care for emergent issues whenever and wherever we’re needed.

In some healthcare settings, it’s common to be directed to an answering service after-hours. In other settings, there are contract workers brought in to cover emergencies.

When you call Crossroads, you get Crossroads. We staff our offices 24 hours a day, so if you need help at 2 a.m., we have a dedicated team of Crossroads staff ready to step in to meet patient needs. This includes both clinical and emotional support staff to ensure we have the right person for whatever the situation requires. 

If a patient needs to be assessed or admitted to hospice care, our Quick Response Team is available when you need them to be.

Once a patient is admitted to hospice care with Crossroads, they are assigned a care team consisting of nurses, aides, a social worker, a chaplain, a bereavement coordinator, and volunteers who all work together under the supervision of our medical director to support the patient and their family.

Over the past 25 years, Crossroads has built a reputation as one of the nation’s leaders in daily visits, providing 16% more care than the national average.

Crossroads is also known for our unique quality-of-life programs including our Veteran Recognition program, our Life Journal program, and our Gift of a Day, providing patients with one perfect day tailored to their interests and wishes.

When a patient begins to decline, our Watch Program is activated, adding additional daily visits so we can monitor the patient’s symptoms.
