What happens after you contact a North Georgia hospice care provider?

As a family member or loved one, you don’t have to wait for a doctor to recommend hospice. In fact, you can contact a hospice care provider yourself.

Generally speaking, the process for contacting Crossroads works like this:

  • As part of your fact finding, one of our experts will answer your questions and address any concerns you may have.
  • We will then schedule a time for a hospice professional to visit the patient, to assess the level of need.
  • If the patient is eligible for hospice, Crossroads will provide medical supplies, equipment and ongoing services.
  • Care will be administered in conjunction with support provided by the patient’s primary care physician.
