Additionally, studies show that Crossroads provides more visits to patients in their final week than the national average for hospice providers.

Is hospice a place or an in-home service?

Contrary to popular belief, hospice is not a place. Hospice is a type of care received by patients with life-limiting illnesses. Hospice service can be administered wherever it is most convenient for the patient – be it their home, a hospital, or an assisted living facility.

What happens after you contact a North Georgia hospice care provider?

As a family member or loved one, you don’t have to wait for a doctor to recommend hospice. In fact, you can contact a hospice care provider yourself.

Generally speaking, the process for contacting Crossroads works like this:

  • As part of your fact finding, one of our experts will answer your questions and address any concerns you may have.
  • We will then schedule a time for a hospice professional to visit the patient, to assess the level of need.
  • If the patient is eligible for hospice, Crossroads will provide medical supplies, equipment and ongoing services.
  • Care will be administered in conjunction with support provided by the patient’s primary care physician.

If you have any questions about hospice services in North Georgia and want to speak with someone now, we are always available. You can complete the form above or reach out directly to us via phone or chat. We’re here for you.
