What are Crossroads’ Hospice Care Link Programs?
These three hospice services programs meet the needs of patients with end-stage cancer, heart disease, or lung disease.
Each of our hospice care programs has been specifically designed to manage the pain and symptoms of patients suffering from cancer, lung disease, or heart disease. With a staff trained to anticipate patients’ unique needs, they will see the immediate benefits of entering one of our unique programs.

Disease Management Programs from Crossroads Hospice:
Our hospice services programs include:
- HeartLink: An end-stage cardiac care program designed to offer end-of-life care to patients with cardiovascular ailments, including congestive heart failure.
- CancerLink: A cancer care program designed by Crossroads Hospice to manage the specific needs of oncology patients with an emphasis on managing the challenges of cancer diagnosis, both physically and emotionally.
- LungLink: A program for patients who wish to withdraw from life-sustaining ventilator support. In particular, this program is for patients diagnosed with end-stage COPD or lung cancer.
